Durham Kids Dentistry is committed to safeguarding the personal information of every child in our care and the parents and guardians who trust us with that care. This policy explains the kinds of information we collect, why we collect it, and the choices you have. Every team member who handles personal information is trained on the sensitivity of these records and on the safeguards we apply to them.
Contact information we collect
In the course of providing pediatric dental care we collect the child's name, the parent or guardian name, your home address, telephone numbers, and email address. We use this information to open and update your child's patient file, to invoice and process payments, to submit pre-authorized insurance claims, to send appointment reminders or treatment follow-ups, and to coordinate with your child's family physician where that is appropriate.
Medical and dental history
We record health details that are relevant to safe pediatric dental care, including physical and mental health background, family history, and dental history. This information helps us diagnose, plan treatment, and avoid medication or material interactions. With your consent, it may be shared with another health or dental care provider for a second opinion, for a referral, or for follow-up treatment. It may also be shared with a third-party insurer or government program in support of claim reimbursement when you have authorized that release.
Confidentiality of medical history
We do not, under any circumstance, supply an insurer with your child's confidential medical history. If we receive an unusual or broad request, we will forward the request to you directly so that you can decide what to release. Where the release seems inappropriate, we will say so.
Financial information
We collect billing details, insurance plan information, and payment data only to the extent needed to bill for services and facilitate insurance reimbursement. Full payment is due at the completion of each procedure.
Future use and retention
Records are retained for as long as required to provide continuing care and to meet professional and statutory obligations. If a future sale or transfer of the practice were ever contemplated, qualified prospective purchasers might be granted limited due diligence access. In that scenario we would require the prospective purchaser to apply the same protections that we apply today.
Regulatory access
The Royal College of Dental Surgeons of Ontario regulates our profession and, as part of its oversight role, may inspect records and interview our team. We cooperate with regulator requirements while continuing to protect patient information.
Cookies and analytics
The website may set basic cookies and use third-party analytics tools to count visits and improve the user experience. These tools report aggregate traffic patterns and do not identify individual visitors. You can clear cookies in your browser at any time.
Compliance
Our handling of personal information follows the federal Personal Information Protection and Electronic Documents Act (PIPEDA) and Ontario's Personal Health Information Protection Act (PHIPA).
